New Member Form
All clubs are asked to report newly recruited members on a monthly basis so that we can (1) place your new member on our Quotarian mailing list as well as our Quota e-Bulletin e-mail list, (2) have the most accurate record of your club’s membership, (3) welcome your new member with a personalized letter and welcome package of materials, and (4) send a note of thanks to the current club member who sponsored your new recruit.

Membership Information Change Form
Thank you for ensuring that your club’s membership records are accurate. Please use the Membership Information Change form to report any changes in a member’s mailing address or status. To print a copy of this form, click here*.

Member-at-Large Form
Members in good standing who relocate to a new geographical location without a club automatically receive member-at-large status. Please use the Membership Information Change form described above to report the new mailing address for qualifying club members who are relocating to a new community without a club. Member Transfer Form
Quota members who transfer to clubs in new locations must have the current and new club president complete a Member Transfer form. To print a copy of this form, click here*.
